PPO USA was formed in 1997 as a division of Government Employees Health Association, Inc. (GEHA). GEHA is the second-largest national health plan for federal employees, retirees and their dependents, with nearly 1 million insured lives. GEHA was incorporated as a not-for-profit member association in 1937 to provide health insurance to Federal Employees.

PPO USA markets access to the managed care networks developed and/or organized by GEHA. Included in the managed care networks available from PPO USA is the PPO USA Medical Network, CONNECTION Dental, CONNECTION's Avesis Vision Plan and CONNECTION Hearing by HearPO.
