1) Completing an online provider search;
2) Calling (877) 254-5481;
3) Looking at your printed medical directory.

Please note:

  1. Some providers may not participate in our network at all locations in which they practice. Before receiving services, it is the member's responsibility to verify that the provider is a participating provider at the location the member receives services.


  2. Some health plans offer the PPO USA Medical network in some states, but not in others. Although you may view a listing of PPO USA Medical network providers in your state, your health plan may or may not contract with the network in your state. Before receiving services, please verify with your health plan that it does offer the PPO USA Medical network in your state.
