You can find providers in your area that currently participate in the PPO USA Medical network by:

1) Completing an online provider search.
2) Calling (877) 254-5481 or
3) Looking in your printed medical PPO directory.

Please note:

1. Some providers may not be participating PPO providers at all locations in which they practice. Before receiving services, it is the member's responsibility to verify that the provider is a participating PPO provider at the location the member receives services.



2. Some health plans offer the PPO USA Medical preferred provider network (PPO) in some states, but not in others. Although you may view a listing of PPO USA Medical network providers in your state, your health plan may or may not contract with the network in your state. Before receiving services, please verify with your health plan that it does offer the PPO USA Medical network in your state.
